Spinal Manipulation & Dynamic Mobilisation – Level 2
Theoretical Content (1 Hour)
K-Theorem Pillar: Performance Pyramid
👉 This course currently includes theoretical content only.
📅 Registration for face-to-face practical classes will be available soon. Stay tuned!
1. Course Overview
This advanced course builds on foundational mobilisation techniques by introducing:
- Refined segmental assessment
- High Velocity Thrust (HVT) techniques
- Manual therapy integrated with functional movement
You’ll learn to evaluate stiffness or restriction in real-world tasks and apply neurophysiological reasoning to unlock functional change—not just range.
💡 Mobility without meaning is wasted. The best therapists use hands-on skill to drive functional outcomes.
2. Learning Objectives
By the end of this theory session, you will be able to:
- Interpret spinal joint behaviour using refined R1/R2 reasoning
- Apply mobilisation Grades 1–5 (including HVT) with accuracy
- Integrate joint mobilisation with movement-based tasks
- Distinguish between restriction, guarding, and motor control issues
- Apply a neurophysiological framework for treatment decisions
3. Core Theoretical Concepts
A. Refined R1 and R2 Profiles
- Early stiff R1, empty R2 → guarding, sensitivity, fear inhibition
- Delayed R1, abrupt R2 → capsular restriction or structural block
- Smooth R1–R2 progression → segment functional; focus on control/strength
👉 Read the joint’s story through your hands—adapt your response accordingly.
B. Mobilisation Grades and HVT in Advanced Practice
| Grade | Range | Application |
|---|---|---|
| 1–2 | Pre-R1 | Calm nervous system, acute/irritable presentations |
| 3–4 | R1–R2 | Restore range, reduce restriction, enhance proprioception |
| 5 (HVT) | At R2 | Quick neuro-mechanical reset; only when clinically indicated |
💡 HVT = fast, low-amplitude input at end-range → restores mobility and segmental reset.
4. Joint Mobilisation With Movement
Combine manual therapy with functional tasks to test treatment relevance.
Examples:
- Thoracic glide + squat → does mobilising T7 improve overhead squat/rotation?
- Lumbar PA glide + bridge → does treatment improve glute activation/control?
Why it matters:
- Connects passive techniques to active goals
- Confirms if the segment drives the deficit
- Aligns with neurophysiological reasoning
5. Clinical Reasoning Shift
| Patho-Anatomical | Neurophysiological |
|---|---|
| Focus on tissue damage or structural fault | Focus on system behaviour & motor response |
| Treats restricted/painful area | Treats what is relevant to functional change |
| Asks: “What’s painful?” | Asks: “What movement/activity is restricted?” |
👉 Best reasoning doesn’t stop at pain—it treats the driver of dysfunction.
6. HVT (High Velocity Thrust): Principles and Clinical Use
Used For:
- Clear end-range restriction
- Rapid mechanical reset
- Segmental inhibition (athletic/high-function clients)
Avoid When:
- Neurological signs or instability
- Unclear diagnosis
- Client apprehension or red flags
Therapist Language Example:
“This technique is fast and controlled. It helps reset a stiff joint and may improve your movement instantly. You might hear a click—that’s normal, but not the goal.”
7. Integration With Functional Retraining
Always reinforce mobilisation with movement:
- Breathing + spinal segmentation drills
- Loaded hip hinge, rotation, gait mechanics
- Balance or split stance integration
- Strength + control retraining for sport/task
8. Clinical Scenarios and K-Theorem Connection
| Scenario | Application | K-Theorem Pillar |
|---|---|---|
| Olympic lifter with limited overhead ROM | Mobilise T6–T8 with squat/press integration | Performance Pyramid |
| Field sport athlete with SIJ pain | Mobilise L5–S1; reassess lunging/sprint pattern | Performance Pyramid |
| Swimmer with shoulder impingement | Mobilise CT junction + scapular repositioning during reach | Performance Pyramid |
| Cervicogenic headache | Mobilise C0–C2; reassess neck rotation & suboccipital tone | Performance Pyramid + Mind |
9. Case Reflection Prompt
- “Did your mobilisation influence the client’s meaningful movement pattern?”
- “Was your decision based on pain, or function?”
✅ You’ve completed the theoretical section.
