👉 This course currently includes theoretical content only.
📅 Registration for face-to-face practical classes will be available soon. Stay tuned!
1. Course Overview
This course introduces foundational skills for assessing and treating spinal joints.
You’ll learn how to:
- Assess segmental motion using R1 and R2 principles.
- Identify stiffness or hypermobility.
- Apply safe and effective joint mobilisation techniques.
The emphasis is on hands-on skill, clinical reasoning, and understanding how joint mobility influences the nervous system, posture, and movement control.
💡 The more joint range there is, the more the brain can access this movement.
2. Learning Objectives
By the end of this theory session, you will be able to:
- Understand the principles of R1 and R2 in joint assessment.
- Differentiate joint hypomobility from hypermobility.
- Grade joint mobilisation techniques (Grades 1–5) based on amplitude and resistance.
- Apply treatment grades for pain modulation vs mobility restoration.
- Explain how spinal mobility affects global movement patterns and brain-body connection.
3. Core Theoretical Concepts
A. R1 and R2 in Spinal Assessment
- R1 (First Resistance): The point where movement starts to resist—often neural or capsular.
- R2 (End Range): The anatomical limit of motion—usually muscular, ligamentous, or structural.
- All spinal segments (cervical, thoracic, lumbar) should be assessed actively and passively at both physiological and accessory levels.
Key Interpretation:
- R1 ≠ R2 → restriction or guarding.
- Ideal: R1 is smooth and close to R2 → functional joint.
B. Joint Grades for Mobilisation
| Grade | Application | Range | Purpose |
|---|---|---|---|
| Grade 1 | Pre-R1, small amplitude | Pain relief | Neuro-modulation |
| Grade 2 | Pre-R1, large amplitude | Pain relief | Calming overactive systems |
| Grade 3 | Into and out of R1, towards R2 | Increase range | Improve motion tolerance |
| Grade 4 | Between R1 and R2, small amplitude | Refine stiffness | Improve segmental control |
| Grade 5 | High velocity thrust at R2 | Reset joint position | Quick mechanical input |
4. Assessing the Spine
- Test each segment for quality, end feel, and symptom reproduction.
- Perform both:
- Physiological movements: flexion, extension, rotation.
- Accessory movements: PA glides, transverse glides.
- Identify:
- Hypomobility: Stiffness, loss of segmental contribution.
- Hypermobility: Excess range, may require stabilisation or control work.
💡 Assessment helps you choose the right grade and technique.
5. Treatment Decision-Making
- Pain-dominant presentations: Use Grades 1–2 (stay pre-R1 to desensitise the nervous system).
- Stiffness-dominant presentations: Use Grades 3–4 (restore movement range).
- Grade 5 (HVLA): Reserved for trained professionals when clinically indicated.
👉 You are restoring the joint’s language to the brain—unlocking better movement, posture, and coordination.
6. Communicating With Clients
“Each spinal segment contributes to how your body moves. When a joint is stiff, the brain stops using it. Mobilising the joint helps the brain reconnect, reducing pain and improving control.”
Clients should understand the link between spinal motion and:
- Pain relief
- Posture
- Movement fluidity
7. Therapist Philosophy
- Mobilisation isn’t about force—it’s about clarity.
- Your hands are both diagnostic and therapeutic.
- Each joint has a story: find it, listen, then treat with purpose.
8. Clinical Scenarios and K-Theorem Connection
| Scenario | Application | K-Theorem Pillar |
|---|---|---|
| Desk-based client with upper neck pain | Assess C2–C4 segmentally, apply Grade 1–2 mobilisations | Performance Pyramid |
| Athlete with reduced lumbar rotation | Use accessory PA glides on L4–L5, Grade 3–4 to restore movement | Performance Pyramid |
| Adolescent with hypermobility and pain | Identify unstable segments, avoid mobilisation, teach control strategies | Performance Pyramid |
9. Case Reflection Prompt
- “What does it tell you if R1 is early and stiff, but R2 feels inhibited?”
- “How do you decide when to treat with Grade 2 versus Grade 3?”
✅ You’ve completed the theoretical section.
