Clinical Skills for Physiotherapy Students – Level 1
Theoretical Content (1 Hour)
K-Theorem Pillar: Performance Pyramid, Movement Matrix
👉 This course currently includes theoretical content only.
📅 Registration for face-to-face practical classes will be available soon. Stay tuned!
1. Course Overview
This course is designed to help physiotherapy students build confidence in performing consistent, clinically reasoned assessments.
You’ll learn how to apply the Elite Akademy Subjective and Objective Frameworks, which guide every client interaction—from the first question to the final re-test.
The emphasis is on clarity, confidence, and communication. Assessment is not just a checklist—it’s the story you build to guide treatment.
2. Learning Objectives
By the end of the theory session, you will be able to:
- Perform a structured, clinically reasoned subjective assessment.
- Apply a step-by-step objective assessment that links to treatment.
- Understand how to identify meaningful impairments and drivers.
- Use clear clinical language to guide client interactions.
- Re-test meaningfully to track outcomes and measure progress.
3. Core Theoretical Concepts
A. Subjective Assessment Framework
Follow the 8-step Elite Akademy process:
| Step | Focus | Why It Matters |
|---|---|---|
| 1 | Presenting Complaint | Understand the patient’s main concern in their own words. |
| 2 | General Information | Profile risk, age, occupation, dominant side, expectations. |
| 3 | Pain / Symptom Description | Location, type, behaviour — key for diagnosis. |
| 4 | Functional Impact | Define what they can’t do — this sets your goals. |
| 5 | 24-Hour Behaviour | Tracks mechanical vs inflammatory patterns. |
| 6 | Previous History & Investigations | Guides clinical reasoning and avoids duplication. |
| 7 | Red & Yellow Flags | Screens for pathology, fear, or psychosocial barriers. |
| 8 | Goals & Expectations | Aligns treatment direction with patient values. |
💡 Use open-ended questions. Listen with intent. Extract functional meaning.
B. Objective Assessment Framework
Follow the 13-step Elite Akademy process to uncover movement, strength, joint, neural and system impairments:
| Step | Focus | Key Purpose |
|---|---|---|
| 1 | Observation | Detect asymmetries, guarding, and posture. |
| 2 | Functional Tests / Movement Analysis | Recreate tasks to identify compensations or imbalances. |
| 3 | Special Tests (If Applicable) | Confirm or rule out structural pathologies. |
| 4 | Neural Dynamics Testing | Identify neural restrictions using R1/R2 model. |
| 5 | Palpation | Confirm localised tension, guarding, or trigger points. |
| 6 | AROM with Overpressure | Assess active range and symptom response. |
| 7 | PROM with Overpressure | Identify capsular or protective patterns. |
| 8 | MST – Manual Strength Testing | Find weakness, inhibition, or guarding. |
| 9 | MLT – Muscle Length Testing | Detect tissue adaptions or flexibility issues. |
| 10 | PAIVMs – Passive Accessory Intervertebral Movements | Joint stiffness, hypomobility, and segmental faults. |
| 11 | PPIVMs – Passive Physiological Movements | Flexion/extension/rotation tests for spinal motion. |
| 12 | Regional Area Checklist | Screen linked areas (spine, hip, shoulder, etc.). |
| 13 | Neuro Screening (if indicated) | Dermatomes, reflexes, and myotomes. |
💡 Always link back to function: “How does this impairment relate to what they can’t do?”
4. Consistency, Reasoning, and Re-Testing
- Don’t treat what you can’t measure.
- Use clear re-test points (e.g. pain-free squat, AROM, strength test).
- Re-test early, and often, to confirm if treatment worked.
- Consistency builds trust—with your clients and your clinical team.
5. Clinical Communication Examples
“You’ve told me your main goal is to return to running pain-free. Today’s assessment will help us find what’s stopping you—and we’ll re-test to see change.”
“Your knee isn’t weak—it’s being guarded by the system. Let’s test strength and neural tension to find the cause.”
6. Clinical Scenarios and K-Theorem Connection
| Scenario | Application | K-Theorem Pillar |
|---|---|---|
| Student unsure where to begin assessment | Rule out cervical or thoracic contribution using the checklist | Performance Pyramid |
| Post-op shoulder patient with lingering pain | Clarify goals, functional limitations, and rule out red/yellow flags | Performance Pyramid |
| Client with vague symptoms, multiple areas | Clarify goals, functional limitation, and rule out red/yellow flags | Movement Matrix + Mind |
7. Case Reflection Prompt
- “What part of the subjective assessment gave you the clearest treatment direction?”
- “How did your objective findings link to the client’s main complaint?”
✅ You’ve completed the theoretical section.
