Advanced Clinical Skills for Physiotherapy Students – Level 2

Theoretical Content (1 Hour)
K-Theorem Pillar: Performance Pyramid, Movement Matrix, Nutrition, Mind

👉 This course currently includes theoretical content only.
📅 Registration for face-to-face practical classes will be available soon. Stay tuned!

1. Course Overview

This course builds on your Level 1 foundations in subjective and objective assessment.

In Level 2, the focus shifts to:

  • Clinical reasoning
  • Prioritisation
  • Managing complexity

You’ll learn how to:

  • Distinguish between symptoms and causes using a neurophysiological approach.
  • Structure reasoning with the Hoshin Diagram.
  • Apply principles from sports rehab, nutrition, and psychology to improve outcomes and athlete engagement.

💡 Great clinicians don’t just assess—they interpret, prioritise, and lead with clarity.

2. Learning Objectives

By the end of this theory session, you will be able to:

  • Apply the neurophysiological model to guide treatment focus.
  • Use the Hoshin Diagram to prioritise multiple assessment findings.
  • Integrate return-to-sport frameworks into planning.
  • Recognise the influence of mindset and nutrition in clinical recovery.
  • Communicate reasoning clearly to both clients and other professionals.

3. Neurophysiological vs Patho-Anatomical Reasoning

Patho-Anatomical ModelNeurophysiological Model
Focuses on the painful tissueFocuses on what drives the dysfunction
Treats structure (e.g., tendon, joint, muscle)Treats system (e.g., tone, control, sensitivity, load response)
Asks: “What is damaged?”Asks: “What is behaving abnormally?”
Is often reactiveIs proactive and pattern-based

💡 A neurophysiological approach finds the solution, not just the symptom.

4. The Hoshin Diagram: Structured Clinical Reasoning

A. What It Is

The Hoshin Diagram is a visual decision-making tool that maps relationships between dysfunctions to identify what drives the problem.

Originally from business process engineering, it now supports clinical reasoning by helping therapists prioritise effectively.

B. Application (Case Scenario Example)

Problem List:

  • Ankle dorsiflexion stiffness
  • Hip IR restriction
  • Single-leg glute bridge deficits
  • Single-leg heel raise weakness
  • Hip abduction weakness
  • Single-leg squat control

C. Step-by-Step Process

  1. Draw each issue in a grid.
  2. Draw arrows based on cause-effect relationships (what drives what).
  3. Count outgoing arrows → shows which issue influences others most.
  4. Prioritise based on total outgoing arrows.
  5. Use this list to determine your first treatment point.

D. Why It Works

  • Provides a repeatable process under pressure.
  • Involves clients in the logic.
  • Tests cause-effect relationships through intervention.
  • Helps athletes “see the plan” and trust the process.

👉 It’s not guesswork. It’s systems thinking applied to human movement.

5. Return-to-Sport & Rehab Frameworks

StageGoalWhat You Do
Return to BaselineRestore movement, control, capacityMobility, neural loading, strength
Return to TrainingControlled exposure to load & speedMonitor RFD, asymmetry, pain-free patterns
Return to SportFull speed, full load, psychological readinessRe-test under fatigue, chaos, pressure

💡 Objective testing + athlete mindset = readiness.

6. Nutrition: The Overlooked Variable

  • Tissue healing, inflammation control, and energy production all depend on gut health and nutrient timing.

Clinical flags:

  • Slow healing → check protein, collagen, iron, zinc, Vitamin D.
  • Poor sleep/fatigue → review hydration, B-vitamins, magnesium.
  • Skipping meals/dieting → increased injury risk.

👉 “You can’t rehab what you don’t nourish.”

7. Mindset & Recovery Psychology

Mindset DomainClinical Link
ConfidenceWillingness to load, participate, push through fatigue
Fear avoidanceGuarding patterns, symptom magnification
Emotional stressDelayed healing, sleep disturbance, higher irritability
Goal clarityCompliance, recovery rate, return-to-sport engagement

Clinical Strategies:

  • Use reframing language: “Your body’s not broken—it’s adapting.”
  • Highlight small wins weekly.
  • Collaborate on planning, not just instruction.
  • Recognise and refer for deeper psych support when needed.

8. K-Theorem Integration Summary

K-Theorem PillarApplication in Level 2 Clinical Skills
Performance PyramidPrioritise movement efficiency, segmental control, neuromuscular stability
Movement MatrixAssess and adapt for daily patterns, stressors, lifestyle inputs
NutritionOptimise recovery, energy, tissue health
MindPrioritise movement efficiency, segmental control, and neuromuscular stability

9. Case Reflection Prompt

  • “Which test result led to the biggest downstream improvements?”
  • “Did I treat the cause, or just respond to the pain?”
  • “How well does my client understand the why behind the plan?”
